Forms for personal trainers and online coaches
Two signatures should exist before any client's first session: the health screen (PAR-Q+) and the liability waiver. A third, the training agreement, decides what happens when a client cancels late, stops showing up, or asks for a refund on an unused package.
The pack below adds the assessment tracker and program sheet that make progress visible at renewal time, and a separate agreement for online coaching clients.

Questions
Is a personal trainer waiver enforceable?
Usually, if it clearly describes the risks, names ordinary negligence and is conspicuous. Some states limit waivers, so have the wording reviewed locally. Not legal advice.
Do online clients need a different contract?
Yes. Online coaching has a minimum term, check-ins and unsupervised training, which the in-person agreement does not cover.
Is the PAR-Q+ free?
The PAR-Q+ screening questions are widely available. The pack adds the physician clearance letter, health details and goals section, and ties it to the waiver and agreement.
