Forms for salons, barbershops and spas
A salon's paperwork protects two things: the client's safety before a chemical or skin service, and the owner's business when chairs are rented to independent stylists. The first is the intake and consent; the second is the booth rental agreement.
The pack below includes both, plus the policies handout that stops no-show arguments, a color record card, a price list, gift certificates and the sanitation log your state board inspector will ask for.

Questions
Does a booth rental agreement make a stylist an independent contractor?
Paper alone does not. Classification depends on control in practice: who sets hours, prices and clients. The agreement is written around those factors; check your state's rules.
Do I need a consent form for hair color?
Many salons use one for first-time color and lightening, and some states require a patch test record for tints. It documents disclosure and the patch test if a reaction happens.
Can clients sign on a tablet?
Yes. Use the PDF versions in any PDF signing app.
