Tattoo Aftercare Instructions
Wrap or adhesive film instructions, first wash step by step, days 2 to 14, what is normal by week, a do-not table with the reason for each rule, when to contact a doctor, long-term sun care, six questions clients ask, and a next-visit box. Two pages to hand over.

Touch-Up & Cover-Up Policy

- 9
Sterilization & Spore Test Log
Three landscape pages: an autoclave cycle log (load number, contents, temperature, pressure, time, chemical indicator, operator), a spore test record with your testing schedule and a six-step failed-test procedure, and a daily procedure and station log that ties each client to a sterile load. Each page has a shaded example row.
- 10
Client Record Card
Client details, ID verified, an alerts box for allergies and conditions, photo permission, and a procedure history with ink brand, color and lot number or jewelry material and lot, needle lots, pouch load number and price. Two example rows.
What you can count on
- Health questionnaire as 16 yes/no questions, so nothing gets skipped with a single "no medical issues" line
- Spelling and stencil sign-off: the client prints the lettering on the form and initials that they checked it
- Every risk and aftercare statement has its own initials box, plus a release that does not try to waive negligence
- Minor piercing consent with guardian ID and proof-of-relationship checks, and no specific law stated
- Ink and needle lot numbers on the client record and pouch load numbers on the daily log, so a recall or failed spore test traces to clients
- Studio name, permit number, deposit terms, notice periods, touch-up window and healing ranges are yellow [placeholders]
- Black and charcoal studio design that prints cleanly on any office printer; signature blocks never split across pages

Before you buy
Are these consent forms legal in my state?
They are written to be state-neutral and do not cite any law. Body art is regulated by states, counties and cities, so the Start Here guide lists what to check with your health department: minimum ages, accepted ID, required consent wording, aftercare and record keeping. They are templates, not legal or medical advice. Have a local attorney review the release and deposit terms before you rely on them.
Can I tattoo minors with this pack?
No. There is a minor consent for piercing only, with a parent or guardian section, because tattooing minors is restricted or prohibited in many places. Check your state and local rules before piercing anyone under 18; the form has a box for your studio's own age policy.
Can clients sign on a tablet?
Yes. Open the PDF in any tablet signing app, or edit the Word file and export it to PDF. Tick boxes, initials boxes and signature lines all work in a signing app or on paper.
Can I edit them in Google Docs or Pages?
Yes. Upload a .docx to Google Drive and choose Open with Google Docs, or open it in Apple Pages. The files use standard tables, text tick boxes and highlighting, with no macros. The sterilization log is landscape; the other forms are US Letter portrait.
Is the aftercare medical advice?
No. The aftercare sheets are general guidance written as a starting point. Healing times and bandage times are yellow placeholders so you can make them match what your artists and piercers tell clients and what your health department requires.

Instantly after payment: a download page opens and the link is also emailed to you. Lost it? Email us and we resend it.
